Applications are taken by Appointment Only: Contact the Putnam County Sheriff's office at 419 523-3208 to set up an appointment. Appointments can be made Monday - Friday 8:30 - 4:00.

Fees: New Issue Ohio residents for five consecutive years or more $67.00 New Issue Ohio resident less than 5 years $91.00 ($67 application fee plus $24 for FBI record check) Renewal: $50.00 Renewal resident for less than 5 years: $74.00 Add $3.00 to fees if you do not supply your own current passport picture for application Fees are to be paid by Cashiers check or Money order made payable to the Putnam County Sheriff's Office. Personal Checks and Cash WILL NOT BE ACCEPTED. All fees are non-refundable Fees are waived to all retired law enforcement.
